Dental
You have two MetLife Dental plans to choose from. The MetLife Dental plans give you access to providers through a national dental network. The MetLife plan will accept both in-network and out-of-network providers; however, your dental dollars will go further when choosing an in-network dentist. By selecting an in-network MetLife provider, deeper negotiated discounts will apply.
Dental coverage is a separate election; therefore you can choose dental coverage even if you don’t enroll in a medical plan through Inspire. For more information, visit metlife.com.
Vision
Inspire offers two vision plans through Vision Service Plan (VSP)—the nation’s leading provider of vision coverage. Employees and their dependents enrolled in either plan have access to the nation’s network of eye care professionals. VSP also offers you the added flexibility of choosing an out-of-network provider, and will reimburse you up to the amount allowed under the plan. For more information, visit vsp.com.
Basic Life Coverage
Inspire provides a company-paid life insurance benefit to eligible employees.
Department Managers and above: You will receive a benefit in an amount equal to your annual base salary.
Shift Managers and Team Members: You will receive a $10,000 benefit.
Optional Life Insurance & AD&D*
Optional Life Insurance
Beyond the Basic Life insurance benefit provided by Inspire, you may elect additional coverage for yourself and your dependents.
- For yourself: You may purchase Optional Life insurance in increments of $10,000 up to $1,000,000.
- For your spouse: You may purchase Optional Life Insurance for your spouse in increments of $10,000 up to $250,000, but no more than 100% of your combined Basic and Optional insurance.
- For your Child(ren): You may purchase Optional Life Insurance for your eligible children in the amounts of $1,000 for children age 14 days to 6 months, and $10,000 for children ages 6 months to 26 years.
Accidental Death & Dismemberment
If you are involved in an accident that is covered under the plan, you or your surviving beneficiary will receive an Accidental Death and Dismemberment (AD&D) insurance benefit. This benefit is in addition to your Basic Life insurance coverage, and provides another amount equal to your annual base salary up to a $1,000,000 maximum.

Disability Insurance*
Short-Term Disability
Inspire provides company-paid Short-Term Disability benefits to full-time management and administrative employees who have one year of continuous service in an eligible position. Eligibility for Short-Term Disability pay for a non-work-related accident or illness begins on the first day of the absence, but only for absences greater than six days. Should you become physically unable to work due to a medically-diagnosed condition, you may receive up to 26 weeks of Short-Term Disability benefits at 70% of base pay in any given 12-month period.
Long-Term Disability
Inspire provides company-paid Long-Term Disability (LTD) benefits to full-time management and administrative employees on the first of the month following 30 days of service in an eligible position. Should you become unable to work after 26 weeks due to a medically diagnosed condition, you are eligible to apply for LTD benefits. Under the plan, Reliance Standard will pay 60% of your income—for as long as you continue to meet the plan’s definition of disability—up to the normal Social Security retirement age. Income is defined as annual base salary.

Administered by CorpCare Associates, this confidential service gives you free access to fully-licensed and certified counselors who can help you navigate through professional, personal, family issues and other life challenges.
